TAB Team Production Bonus Acknowledgement Form

• The form is generated for Members when they achieve their first month GET Team qualification

• Home Office send the form to Member Services, who send it to the Member

• The Member must sign and return the form (post or scan and e-mail) to Herbalife as soon as the third month qualification has been completed

• If we don’t receive the Acknowledgement form, we will hold the first month’s earnings

• If we don’t receive the Acknowledgement form by the 2nd month, we will pay the earnings up to the appropriate qualified upline TAB Team Member
